You agree that the information given is a true and accurate record. You or the responsible party, agree to be responsible for payment of fees at the time of service. Any expenses, costs of disbursements incurred by Cranbourne North Dental shall be paid by the responsible party listed in the details I have given.
In the event where your overdue account is referred to a collection agency and/or law firm, you will be liable for all costs which would be incurred as if the debt is collected in full, including legal demand costs.
In some instances, a deposit/pre-payment will be required and will be advised accordingly. Failure to attend any appointment or cancellation without sufficient notice may result in this deposit being forfeited and/or a fee being charged and a further deposit requirement for any future appointments.
Surcharges will be applied to all Credit Card and any Contactless payments (including tapping with a debit card). Any refunds processed will be issued less the surcharge. If a Payment Plan is used, refunds of unused deposits will be adjusted to account for any applicable pro-rata payment plan fees.
Use of AI for Documentation
By attending appointments, you consent to the recording of consultations for the purpose of creating transcriptions of patient notes, referrals, or other documents through AI software. All transcriptions undergo review by our qualified clinicians to ensure accuracy and compliance with applicable privacy laws and regulations. These recordings and transcriptions are securely stored and managed in accordance with relevant data protection standards.
Call Recording
Telephone calls with our team may be recorded for quality assurance, training, and service improvement purposes. All recordings are stored securely and used exclusively for these purposes in strict compliance with applicable privacy laws and regulations.
CCTV Use
Our premises are monitored by closed-circuit television (CCTV) systems for the purposes of ensuring the safety and security of patients, staff, and property. CCTV footage is securely stored and used only for security-related purposes in accordance with applicable privacy regulations.
While we endeavour to provide patients with a courtesy reminder for their appointments, it is ultimately the patient’s responsibility to remember the appointment time. Our practice requires 48 hours notice for changes for all appointments. Failure to give adequate notice may result in a fee charge.
